Lisp & Speech Impairment
A lisp is often rooted in improper tongue placement, not just a speech habit. Orofacial Myofunctional Therapy corrects the underlying muscle patterns that cause lisping and other speech sound errors.
What is Lisp & Speech Impairment?
A lisp is a speech impairment characterized by difficulty producing certain sounds — most commonly the 's' and 'z' sounds — due to incorrect tongue placement during speech. Rather than the tongue staying behind the teeth, it pushes forward (interdental lisp), to the sides (lateral lisp), or against the teeth (dental lisp), distorting the sounds produced.
From an orofacial myofunctional perspective, a lisp is rarely just a speech issue. It is almost always accompanied by underlying muscle dysfunction — specifically improper tongue resting posture, tongue thrust, or low tongue tone. When the tongue habitually rests in a forward or low position, that same position carries over into speech and produces the characteristic distortions associated with lisping.
Orofacial Myofunctional Therapy and speech therapy are closely related when it comes to treating lisps. OMT addresses the foundational muscle patterns and oral posture that drive the speech errors, making it a powerful complement to — or, for many patients, a standalone solution for — correcting the lisp at its root cause.
Signs & Symptoms
- Distorted 's' and 'z' sounds (sounding like 'th' or a wet, slurred quality)
- Tongue visibly pushing forward between or against the teeth during speech
- Difficulty with 'sh', 'ch', 'j', 'l', or 'r' sounds in addition to 's' and 'z'
- Low or forward tongue resting posture when not speaking
- Open mouth rest posture or mouth breathing
- Tongue thrust swallow pattern
- History of prolonged pacifier use, thumb sucking, or digit habits
- Difficulty being understood by others, especially in noisy environments
- Self-consciousness or avoidance of speaking in social situations
How Orofacial Myofunctional Therapy Helps
Orofacial Myofunctional Therapy corrects the tongue posture and muscle function that underlie most lisps. Before a patient can reliably produce correct speech sounds, the tongue must have the strength, coordination, and postural awareness to consistently land in the right place — and that is exactly what OMT trains.
Therapy begins by establishing proper tongue resting posture: tongue tip resting on the spot just behind the upper front teeth (the alveolar ridge), with the body of the tongue gently suctioned to the roof of the mouth. This foundational posture creates the muscle memory needed for accurate tongue placement during speech. We also address lip seal and nasal breathing, which support the overall oral environment for clear articulation.
Once resting posture is established, therapy progresses to tongue placement accuracy during speech sounds, progressing from isolated sounds to syllables, words, and conversational speech. For patients who are also working with a speech-language pathologist, OMT coordinates directly with their program to ensure the most efficient and lasting results.
Therapy Techniques
- Tongue tip placement drills targeting the alveolar ridge (the 'spot')
- Tongue elevation and strength exercises to improve precision and control
- Tongue lateralization and tip isolation training
- Lip seal and facial muscle coordination exercises
- Nasal breathing awareness to support proper oral rest posture
- Sound-specific placement practice progressing from isolation to conversation
Expected Outcomes
- Corrected tongue placement for accurate 's', 'z', and related speech sounds
- Improved overall speech clarity and intelligibility
- Proper tongue resting posture on the palate between words and at rest
- Reduced or eliminated tongue thrust patterns
- Greater confidence and ease in speaking socially and professionally
- Long-term correction rooted in muscle retraining, not just habit adjustment
Who is this for?
Children, teens, and adults with any type of lisp or speech sound impairment related to tongue placement. OMT is effective as a standalone approach or alongside speech-language pathology services. Early intervention in childhood produces the fastest results, but adults benefit significantly as well.
